Carrier Information Sheet. (1 page)      Be sure it is filled out completely.  Be sure to provide ALL of your contact numbers.  If      we cannot make contact with you, you cannot haul our freight. We make ourselves      available 7 days a week and we expect the same from our carriers. 2. Broker Carrier Agreement (5 pages)      Again, fill it out completely. Initial each page at the bottom and have someone witness      signature on the last page.  3. Current Insurance Certificate listing "Hicks Auto Transport" as additional insured.       This must come directly from your agent.  You must contact them, we do not do      this for you.  4.  Copy of ICC authority.        If this is provided through Central Dispatch then we will download and print it. 5. W-9       Again, if this is provided through Central Dispatch then we will download and print it. 6.  Tie Down Warning Document.     This applies only to CarMax freight, but is required in every carrier file.   Be sure to complete each item and fax the packet back to us at: 321-951-8796.  Be sure to reference the Load# or a description of the freight you are requesting if a Load# has not been provided to you.  ONCE YOUR FILE IS COMPLETE, YOUR LOAD WILL BE DISPATCHED TO YOU.   You have the option of having your dispatch order faxed or emailed to you.  Please advise us of your preference and make sure you have provided your email address. General Information: 1. All carriers are required to deliver on a Bill of Lading with their correct name and address.  2. All Carmax freight requires a signature on delivery.  Any Carmax GMAC/Openlane or Smart Auction units require a release page seperate from your Bill of Lading to be signed at delivery.  Failure to secure a signature will result in non-payment.   This is Carmax's policy and it is strictly enforced.  3.  All carriers are required to obtain at least the last 8 of the vin# on each unit.  DO NOT FAX AN INCOMPLETE BILL OF LADING, NO VINS= NOPAY. 4.  Do a complete inspection upon loading.  Note any damage on the back of the gate pass BEFORE pulling the car out of the auction.  If you are loading at a dealership, have them sign for any damage before leaving. 5. All CARMAX freight must be inspected for PRIOR TIE DOWN DAMAGE.  If you see any rips or tears in the undercarrige STOP AND CALL DAVID AT 321-258-7611 IMMEDIATELY.  IF YOU LEAVE WITH THE VEHICLE YOU MAY OWN IT. 6. Fax the Bill of Laidng on delivery.  Most of the time you can use the fax at the delivering location. Office Fax Number is #  321-951-8796.  Please remember we cannot process your payment without proof of delivery.  Policy for Invoicing: 1. Any load requiring a seperate Bill of Lading will require it's own invoice.  Each invoice MUST HAVE AN INVOICE NUMBER.  If you invoice mutiple customers on one invoice your payment will be processed when all customers have paid.  We do not short pay invoices. 2. Our payment policy is clearly stated in your Broker/Carrier Agreement.  We urge you to read it.  Any payment terms    negotiated must be noted on your invoice.
